Y Diabetes Prevention Program at the Y

YMCA Diabetes Prevention Program

The Diabetes Prevention Program helps adults at high risk of developing type 2 diabetes adopt and maintain healthy lifestyles. Participants will work together in a small group (in person, or through socially-distant platforms like teleconferencing, or virtual meeting platforms)  to learn about healthier eating and increasing their physical activity to reduce their risk of developing diabetes. 

 
  • 1 year (16 weekly, 4 bi-weekly, 6 monthly) for 1 hour  per session
  • Small group discussion facilitated by a YMCA Certified Lifestyle Coach
  • Topics cover Nutrition, Physical Activity and the Mental Aspect of Change
  • Adults must be diagnosed prediabetic or qualify through the 
    “What is your risk score?” assessment
 

The YMCA’s Diabetes Prevention Program is part of the CDC-led National Diabetes Prevention Program.

What is Prediabetes?


Prediabetes occurs when blood sugar levels are higher than normal but not high enough for a type 2 diabetes diagnosis. The risk of developing type 2 diabetes may be reduced or eliminated by weight loss, healthier eating, and increased physical activity.

IF YOU SCORED A 5 OR HIGHER, then you may be at risk for prediabetes or diabetes and may qualify for the program. This does NOT mean you have diabetes. You will need a blood test to confirm if you have diabetes.

Blood Pressure Monitoring at the Y

Blood Pressure Monitoring

The purpose of the Blood Pressure Self-Monitoring program is to assist adults diagnosed with hypertension to lower their systolic and diastolic blood pressure through the simple process of checking and recording their blood pressure at least twice a month over a four-month period. Participants in this program will work with a Healthy Heart Ambassador to:

  • Self-measure their blood pressure at least twice a month

  • Attend two personalized consultations per month

  • Attend monthly nutrition education seminars

  • The Y will provide options for participants who need a home blood pressure monitor.
